Instructions

Review the below form starting with the school/organization and main contact info.
Click the “Please download and fill out the Subscriber Agreement form and upload here.” link.
- The Subscriber Agreement form will download to your computer.
- Open & complete it.
- Print it.
- Upload back to CViConnect, using the “Choose File” button.
If you have less than 20 students, the payment information section will be visible. Please select either credit card or PO, then enter the credit card information or upload a signed PO document.
- If you have 20+ students please go ahead and submit the form and we will contact you shortly for customized pricing.
